About the role

Welcome to Apply. This role is ideal for recent RN graduates and LPNs transitioning into RN careers, focusing on organization, attention to detail, and supporting both resident care and facility outcomes.

Responsibilities

  • Coordinate and complete MDS assessments from start to finish, ensuring accuracy and timely submission
  • Manage daily and weekly MDS schedules (ARDs), tracking due dates, and preventing delays
  • Review resident charts, therapy notes, and nursing documentation to ensure comprehensive assessments
  • Work closely with nursing, therapy, dietary, and social services to gather necessary information and align documentation
  • Lead or participate in care plan meetings to ensure plans reflect residents' needs and goals
  • Identify and address missing or inconsistent documentation, working with the team to resolve issues before submission
  • Maintain accurate documentation to support Case Mix (CMI) and ensure proper reimbursement
  • Assist with audits, surveys, and quality reviews by providing organized, compliant, and survey-ready documentation
  • Support staff in understanding and improving documentation best practices to enhance accuracy and outcomes
  • Act as a key point of contact for all matters related to MDS, care planning, and regulatory compliance

Requirements

  • Current, active Registered Nurse (RN) state license OR LPN actively enrolled in an RN program
  • Long-term care experience preferred
  • Prior MDS Coordinator experience preferred, but training is available for the right candidate
  • Strong written and verbal communication skills
  • Detail-oriented, organized, and deadline-driven
  • Positive, dependable, and team-focused attitude
  • Excellent attendance and reliability
