Job Summary
Join our 5-star skilled nursing facility, Winchester Rehabilitation and Nursing Center, as an MDS Nurse RN/LPN! As a valued member of our team, you will play a crucial role in providing high-quality care to our residents.
Responsibilities
- Conduct and coordinate the development and completion of the resident assessment
- Conduct resident assessments in accordance with current rules, regulations, and guidelines
- Implement resident CAA's
- Participate in facility survey readiness and throughout the survey
- Work with our interdisciplinary care team to develop a comprehensive assessment and care plan for each resident
Requirements
- Current unencumbered State of Massachusetts RN or LPN License
- Two years of MDS and PPS experience
- Care Planning experience
