Responsibilities
- Complete and coordinate MDS assessments in accordance with state and federal guidelines.
- Ensure accurate coding and documentation to support reimbursement and quality measures.
- Collaborate with interdisciplinary team members to develop and update care plans.
- Monitor resident progress and maintain regulatory compliance.
- Participate in care plan meetings and quality assurance programs.
Qualifications
- Current RN or LPN license in Massachusetts.
- Minimum 1 year of MDS experience in a skilled nursing facility preferred.
- Strong knowledge of RAI process, PDPM, and Medicare/Medicaid guidelines.
- Excellent attention to detail, communication, and organizational skills.
- Ability to work independently and as part of an interdisciplinary team.
