MDS Coordinator

Premier Rehab & Healthcare at Burlington is seeking an experienced and dedicated MDS Coordinator to join our team. The MDS Coordinator plays an essential role in ensuring accurate resident assessments, comprehensive care planning, regulatory compliance, and high-quality, resident-centered care. Responsibilities Coordinate the development and completion of comprehensive resident assessmentsComplete MDS assessments in accordance with current state and federal regulationsConduct and coordinate Care Area Assessments (CAAs)Coordinate and support individualized resident care plansCollaborate with nursing, therapy, social services, dietary, physicians, and other members of the interdisciplinary teamEnsure MDS documentation is accurate, complete, and submitted within required timelinesMonitor MDS schedules and regulatory deadlinesParticipate in facility survey preparation and the survey processReview resident documentation for accuracy and completenessCommunicate effectively with residents, families, and interdisciplinary team membersSupport quality improvement and regulatory compliance initiatives Qualifications Current, unencumbered Vermont RN or LPN licensePrevious MDS and PPS experience requiredPrevious care planning experience preferredStrong knowledge of MDS, PPS, and applicable regulatory requirementsExcellent assessment, documentation, organizational, and communication skillsStrong attention to detail and ability to meet deadlinesAbility to work effectively as part of an interdisciplinary teamCommitment to providing high-quality, resident-centered care What We Offer Competitive wagesWeekly payMedical, dental, and vision insuranceShort- and long-term disability insuranceAccrued PTO401(k)6 paid holidaysEmployee education and professional development opportunitiesSupportive work environment Join Our Team!
