About the role
A Registered Professional, or Licensed Practical, Nurse is responsible for delivering optimal resident care through collaboration with all disciplines and the nursing process of assessment, planning, implementation, and evaluation. Directs and guides resident and family teaching and activities of LPN and NA while maintaining standards of professional nursing to promote a safe, caring, home-like environment for the resident and families.
Responsibilities
- Applies knowledge of illness, injuries, and diseases and uses available resources in assessment process.
- Responsible for assessing resident status, intervening to assist the resident to attain or maintain the highest practicable level of physical, mental and psychosocial well being; communicating pertinent information to other members of the interdisciplinary team.
- Proceeds with formulating the resident’s interdisciplinary plan of care.
- Conducts implementation of Interdisciplinary Plan of Care.
- Conducts evaluation of nursing aspects of POC.
- Demonstrates knowledge of State of Ohio Nurse Practice Act, hospital, Norwalk Memorial Home, and nursing policies and procedures.
- Acts as a resource person regarding resident care, specialty care, orientation, and teaching of staff.
- Performs other duties as assigned.
Requirements
Must have a current RN license in the state of Ohio or a current LPN license in the state of Ohio.
Qualifications
- Bachelor’s degree in Nursing preferred.
- Current CPR certification required.
Skills
- Strong communication skills.
- Ability to work independently and as part of a team.
- Knowledge of medical terminology and patient care.

Pay
Salary, with every third week on call commitment.
