About the Role

The MDS Coordinator ensures timely and accurate completion of the RAI process and care management from admission to discharge, adhering to company policies, federal, state, and certification guidelines. This role may also coordinate clinical department information systems and education under the direction of the Director of Nursing and VP of Clinical Reimbursement.

Responsibilities

  • Collaborate with the Interdisciplinary Team to assess resident needs.
  • Provide an interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies.
  • Ensure the Interdisciplinary Team makes decisions on completing additional MDS assessments based on clinical criteria from the RAI User’s Manual.
  • Assist with coordination and management of daily stand-up meetings, including reviewing resident care and setting assessment reference dates.
  • Comply with federal and state regulations for the RAI process.
  • Monitor MDS and care plan documentation for all residents, ensuring medical record support for MDS coding.
  • Maintain current MDS status of assigned residents according to state and federal guidelines.
  • Frequently and accurately enter resident information into computerized MDS programs.
  • Complete accurate MDS coding using medical records, observations, and interviews with staff, residents, and family members.
  • Attend interdisciplinary team meetings, quality assurance meetings, and others to gather information and communicate updates.
  • Assist the Director of Nursing (DON) or designee in identifying significant changes, physician orders, and verbal reports to reflect updates in MDS and care plans.
  • Prepare scheduling and notices for resident care planning conferences, and assist the DON in communicating outcomes to staff, residents, and responsible parties.
  • Stay updated on data entry and computer technology.
  • Submit required documentation electronically to state databases and other entities per company policy.
  • Correct and ensure completion of final MDS, submitting resident assessment data to appropriate state and federal agencies.
  • Assign, assist, and instruct staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required), and clinical computer systems.
  • Maintain confidentiality of necessary information.
  • Perform other duties, responsibilities, and activities as assigned.

Qualifications

  • Graduate of an approved Registered Nurse program and licensed in the state of practice.
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
  • Excellent knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.
  • Thorough understanding of the Quality Indicator process.
  • Knowledge of OBRA regulations and Minimum Data Set.
  • Knowledge of the care planning process.
  • Experience with MDS 3.0 preferred.
