Responsibilities
- MDS/RAC Coordinator: Conducts and coordinates the completion of the resident comprehensive assessment (RAC) which includes the Minimum Data Set (MDS), Resident Assessment Protocols (RAPs), and the RAP summary sheet.
- Interdisciplinary Assessments: Ensures interdisciplinary, comprehensive assessments and care plans are completed using common assessment periods within the times frames established by OBRA regulation.
- Scheduling Interdisciplinary Conferences: Schedules interdisciplinary conferences for review and updating of resident assessment and care plan.
- Monitoring Plans of Care: Monitors plans of care to ensure resident assessments are utilized in the development of an interdisciplinary plan of care reflective of the resident’s current physical, mental and psychosocial needs.
- Training and Supervision: Provides educational training and clinical supervision in resident assessment for appropriate health care workers.
- Data Management: Assists Resident Assessment Coordinators (MDS) with computerized medical data and transmission.
- Documentation: Ensures documentation relative to resident assessment process (RAI) is maintained according to State and Federal regulations and company policy and procedure.
- Communication: Communicates problem identification with resident assessment process promptly to the DON and Executive Director.
- Data Reporting: Conducts and participates in PDPM meetings, reports data collection, trends, QSAP reports, and attends QA meetings.
- Safe Work Habits: Performs all duties in a safe and efficient manner, wears safety equipment as required for the job, reports any safety hazards and/or accidents to supervisor, attends all mandatory in-service meetings, complies with all department and facilities policies and procedures.
Qualifications
- Registered Nurse - California Education
- Experience with State and Federal regulations
Skills
- Good communication and customer service skills
- Able to work well with others and take direction
- Able to work under pressure to ensure timely review, correction and reporting of the payroll
- Assists with account payable and accounts receivable tasks

The actual compensation offered will depend on a wide range of factors including, but not limited to, the selected candidate’s knowledge, skills, experience, training, education, and licensure/certifications (as applicable).
Pay
$55.00 - $65.00 per hour
