Responsibilities
- Provide direct nursing care as necessary.
- Prepare for and participate in facility surveys (inspections) conducted by authorized government agencies.
- Support Quality Assurance and Performance Improvement (QAPI) initiatives to develop and implement corrective action plans for identified deficiencies.
- Coordinate the interdisciplinary team to ensure timely completion of resident assessments.
- Utilize the MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
- Communicate ARD selection for specific MDS assessments to the interdisciplinary team (IDT).
- Complete assigned sections of the MDS and fulfill Care Area Assessments (CAAs) as required.
- Complete Care Plan items and attend Care Plan meetings as assigned.
- Implement a monthly infection tracking log.
- Conclude Utilization of Daily Assessments (UDA’s) during observation periods.
- Finalize diagnosis coding and create diagnosis sheets.
- Confirm skilled physician’s certification.
- Monitor medical record documentation to ensure it supports MDS coding.
- Participate in skilled utilization and State Case Mix management as assigned.
- Provide training and mentoring to interdisciplinary team members and CNA staff on MDS item completion and Nursing ADL documentation.
- Sign off on the Minimum Data Set (MDS) for each resident within the facility.
Requirements
- High School Diploma or equivalent.
- Minimum of one year of experience as a nurse in a hospital, long-term care facility, or related healthcare setting.
- Active RN, LVN, or LPN license valid in the state of employment.
- Experience with the RAI process and MDS certification preferred.
- Must provide evidence of being free of tuberculosis infection upon hire and annually.
- Ability to read, write, speak, and understand English.
- Ability to make independent decisions when circumstances warrant such action.
- Strong interpersonal skills to deal tactfully with personnel, residents, family members, visitors, and government agencies.
- Knowledge of nursing and medical practices, procedures, and regulations pertaining to long-term care.
- Leadership and supervisory ability, with a willingness to work harmoniously with and supervise other personnel.
- Patience, tact, a cheerful disposition, and enthusiasm, along with the willingness to handle difficult residents.
Work Environment
- Works in office areas as well as throughout the nursing service area (e.g., nurse’s stations, resident rooms).
- Subject to frequent interruptions and interactions with residents, personnel, visitors, and government agencies under various conditions.
- May encounter hostile or emotionally upset residents, family members, personnel, or visitors.
- Subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster).
- Exposure to potential hazards, including falls, burns from equipment, odors, dust, disinfectants, tobacco smoke, infectious waste, diseases (including TB, AIDS, and Hepatitis B), and hazardous chemicals.
- May be required to assist in the evacuation of residents during emergencies.
Physical and Sensory Requirements
- Must be able to move intermittently throughout the workday.
- Must be able to cope with the mental and emotional stress of the position.
- Must possess adequate sight and hearing (with or without prosthetics) to perform the requirements of the position.
- Must function independently, with flexibility, personal integrity, and the ability to work effectively under pressure.
- Must meet general health requirements set forth by facility policies, which may include a medical and physical examination.
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and occasionally hostile individuals.
- Must be able to lift up to 25 pounds regularly and up to 50 pounds infrequently; occasional pushing and pulling of carts may be required.
