MDC Social Services Advisor

About the role

The Social Services Advisor serves as a trusted resource and advocate for program participants by assessing needs, developing individualized service plans, connecting clients with community resources, and providing ongoing case management. This position plays a vital role in helping participants achieve greater self-sufficiency through coordinated access to housing, education, employment, healthcare, financial assistance, and other supportive services. The ideal candidate is committed to serving diverse populations, demonstrates excellent interpersonal skills, and has experience working within nonprofit, community-based, or human services organizations.

Responsibilities

• Conduct comprehensive intake assessments to identify participant strengths, needs, and goals.
• Develop individualized service and action plans with measurable outcomes.
• Provide ongoing case management, coaching, and follow-up services.
• Monitor participant progress and adjust service plans as needed.
• Maintain consistent communication to encourage engagement and successful outcomes.
• Connect participants with community resources, including housing, healthcare, mental health services, transportation, childcare, food assistance, financial counseling, and employment supports.
• Advocate on behalf of participants to ensure access to appropriate services.
• Build collaborative relationships with community agencies and service providers.
• Educate participants about available programs and eligibility requirements.
• Coordinate referrals and monitor service delivery.
• Assist with workshops, community outreach events, and participant orientations.
• Collaborate with internal staff to provide comprehensive wraparound services.
• Support implementation of MDC initiatives designed to improve family and community well-being.
• Maintain accurate case files and participant records.
• Document client interactions, referrals, and outcomes in accordance with program and funding requirements.
• Prepare reports and maintain data for grants and organizational reporting.
• Ensure confidentiality and compliance with applicable regulations and organizational policies.
• Represent MDC at community meetings, resource fairs, and partner events.
• Develop and maintain positive relationships with local agencies, educational institutions, employers, and social service providers.
• Promote MDC programs and services throughout the East St. Louis community.

Requirements

• Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Counseling, Public Administration, or a related field.
• OR Associate degree with significant professional experience in social services, case management, workforce development, community outreach, or a related human services field.

Qualifications

• Two or more years of experience in case management, social services, nonprofit programming, workforce development, or community-based services.
• Experience working with underserved individuals and families.
• Knowledge of local and regional social service resources.
• Experience with trauma-informed, strengths-based, or client-centered service delivery.
• Familiarity with grant-funded programs and community partnerships is preferred.

Schedule

Full-time, in-person position located in East St. Louis, Illinois. Standard Monday through Friday work schedule. Frequent interaction with clients, community partners, and service providers. Occasional evening or weekend outreach events may be required. Local travel within the service area may be necessary.

Pay

$58,000 annually
