Maternal Health Supervisor

About the role

The Maternal Health Supervisor is a Family Case Manager responsible for coordinating enrollment with a team of Home Visitors to provide services to mothers within the Maternal Health Home Based Services Program. The role coordinates and/or conducts well baby check-ups and screenings of both the child and the mother during the first 18 months after the child is born.

Responsibilities

• Develop and implement program using evidence based tools
• Coordinate enrollment
• Hire, train and manage direct reports
• Distribute and monitor caseloads
• Develop action plans to monitor and report progress
• Help referred families find community resources such as healthcare, mental health services, housing, financial assistance, employment, transportation, childcare, etc.
• Conduct home visits to identify parenting needs and to provide support for the family and the child's development and well-being
• Encourage a healthy and nurturing lifestyle
• Assist with plans, considering education and assistance finding work

Qualifications

Required:

• Must be at least 21 years of age
• Bachelor's Degree in Human Services or related field and two (2) years' working in a Human Services profession
• Excellent communication skills (verbal and written) with the ability to articulate information to a diverse audience
• Proficient with Microsoft Office Suite
• Current driver's license and the ability to travel for business utilizing personal vehicle

Preferred:

• One (1) year of Supervisory experience
