About the role
Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in multiple counties in New York State. We are seeking a full-time MAT Peer Adherence Worker to join our team at the Peekskill location.
Responsibilities
- Provides recovery support to reduce barriers of care and shares strategies to improve patient health outcomes.
- Edits patients regarding MAT, HCV, risk reduction, medical and medication adherence.
- Shares education resources and materials that promote recovery support education on harm reduction, MAT, Naloxone, and treatment.
- Trains and supports patients in harm reduction and opioid overdose and prevention.
- Engages new patients using harm reduction strategies and supports them in cases of overdose.
- Understands and complies with all HIPPA, OASAS, MAT, and other health-related legal policies and procedures, as well as internal Sun River Health Center policies and procedures.
- Demonstrates respect for everyone’s unique path to recovery.
- Facilitates Naloxone, Fentanyl test strip, Xylazine test strip, and harm reduction training for staff, patients, and the community.
- Aids patients in completing recovery support plans.
- Participates in clinical meetings, quality improvement, case conferences, staff meetings, and supervision.
- Completes electronic and written forms and reports accurately and promptly.
- Participates in ongoing training, supervision, and professional development opportunities.
- Provides health education and adherence support to patients.
Requirements
- Minimum Education: High School Diploma/GED
- Preferred Education: Associate’s degree, Certified Recovery Peer Advocate (CRPA)
- Minimum Work-Related Experience: 1 year
Qualifications
- Knowledge of community resources and ability to link patients to recovery supports and community services.
- Understanding of and compliance with all HIPPA, OASAS, MAT, and other health-related legal policies and procedures, as well as internal Sun River Health Center policies and procedures.
- Respect for everyone’s unique path to recovery.
- Ability to facilitate Naloxone, Fentanyl test strip, Xylazine test strip, and harm reduction training for staff, patients, and the community.
- Ability to assist patients in completing recovery support plans.
- Ability to participate in clinical meetings, quality improvement, case conferences, staff meetings, and supervision.
- Ability to complete electronic and written forms and reports in an accurate and timely manner.
- Ability to participate in ongoing training, supervision, and professional development opportunities.
- Ability to provide health education and adherence support to patients.
