Manager - Case Management

About the role

Responsible for day-to-day oversight and coordination of case management and discharge planning functions to ensure patient care quality and efficiency, while complying with regulatory requirements. Collaborates with multi-disciplinary teams to optimize patient flow and demonstrates critical thinking and effective communication skills. Must work independently in stressful environments and take appropriate actions.

Responsibilities

• Oversee and coordinate case management and discharge planning functions
• Ensure compliance with State and Federal discharge regulations
• Collaborate with multi-disciplinary teams to optimize patient flow
• Apply critical thinking and effective communication in high-pressure environments
• Utilize hospital information systems and performance improvement processes
• Apply Interqual or Milliman guidelines for case management

Requirements

• Minimum of 3 years of case management experience
• Minimum of 2 years of leadership experience in similar roles
• Background in clinical decision-making, medical criteria, and discharge planning
• Nursing degree or BSN required
• Case management certification preferred
• Proficiency with Interqual or Milliman guidelines
• Knowledge of staff development techniques
