Manager Case Management
Job Summary
The Market Manager of Case Management is responsible for supporting the Director of Case Management in managing the Case Management operations. This includes providing leadership for planning, management, implementation, integration and coordination of Case Management in support of facility goals dedicated to excellence, effective leadership, and financial stability. Supporting the delivery of the high quality, cost effective patient care through activities based on facility strategic goals and objectives.
Qualifications
- EDUCATION REQUIRED: BSN or on track for obtaining BSN within 2 years of hire date OR an MSW
- LICENSURE/CERTIFICATION: If RN: Current NH RN license required or appropriate compact licensure.
Essential Functions
- Under the guidance of the Director, the Manager is to assist in the planning and coordination of Case Management activities, operational efficiency and effectiveness as well as major impacts on all other hospital areas.
- Aid the Director in providing direction to the staff of the Case Management team and assist in reviewing and evaluating performance of the department staff.
- Involvement in counseling and appropriate disciplinary actions of staff members as requested.
- Aid department staff in meeting and adhering to all applicable federal, state, JCAHO, and local regulatory agency requirements and assist in the success of the department and facilities in any regulatory survey.
- Make decisions in absence of the director to maintain acceptable operations based on strategic goals and policies.
- Display ingenuity and foresight in determining the most appropriate solution in the absence of established guidelines.
- Confer with other department leaders when necessary to resolve procedural difficulties, clarify department responsibilities, objectives and resolve identified problems.
- Adhere to hospital/departmental attendance policy, work hours and maintenance of personal appearance as stated in facility policy.
- Make suggestions on development of new system processes as needed to support new regulations, software installation, changing environment, and needs of team.
- Work with the Case Management Director to monitor staff caseload, reorganizing department as needed to support patient population on each unit.
- Approve staffing schedule ensuring appropriate coverage to support patient case management needs.
- Maintain daily reports of observation patients.
- Understand financial implications of Length of Stay requirements for organization and serve to improve efficiency while ensuring safe, appropriate discharge planning throughout the organization.
- Collaboratively work with the Case Management team and effectively engage employees, maintaining positive teamwork relationships, and striving to improve employee engagement.
- Develop, coordinate and monitor strategies for obtaining, organizing and allocating resources required for care transitions.
- Assist team members in problem-solving complex patient situations.
- Establish collaborative practice with nursing personnel, physicians, and health care professionals regarding the plan of care and identify variance in plan implementation.
- Identify necessary resources or eliminate unnecessary resources needed to achieve quality patient outcomes as well as advocate for patients facing barriers to resources.
- This includes identifying alternative options if needed resources are not accessible.
- Manage referrals for post-acute services based on Federal and State regulations, client eligibility for service, the patient’s home care needs and the ability of the agency to safely provide the needed services.
- Collaborate with healthcare team, develop treatment goals and plans interventions that assist the patient and family to resolve social and psychosocial concerns that may arise as a result of illness, engagement in high risk social behaviors, limited finances, and lack of family support and/or physical disabilities.
- Collaborate with post-acute discharge providers in the development of transition plans.
- Obtain physician involvement and agreement with plan.
- Advocate for patient choice.
- Provide leadership for multidisciplinary team efforts during interdisciplinary rounds (IDR’s).
- Work collaboratively with peers to achieve department goals and daily work as evidenced by appropriate and timely communication which is respectful and clear.
- Sensitive to workload of team, share responsibilities, fill in and offer help.
- Serve as a preceptor for new hires, facilitate education and growth within the staff, and actively participate in coaching and evaluation of assigned team members.
- Actively participate in conferences, committees and task forces as directed.
- Demonstrate understanding of and adherence to HCA’s Code of Conduct.

- Accept other assignments and duties as assigned.
