Manager - Case Management

Role Overview

Responsible for overseeing daily case management and discharge planning to ensure high-quality, efficient patient care in compliance with regulatory standards.

Key Responsibilities

• Cook up case management activities
• Collaborate with multidisciplinary teams to optimize patient flow
• Ensure discharge processes meet clinical and regulatory requirements
• Utilize clinical decision-making skills, medical criteria, and guidelines such as Interqual or Milliman
• Monitor performance improvements and implement process enhancements
• Respond effectively to stressful situations with minimal supervision

Qualifications & Skills

• Minimum three years of case management experience, including two years in leadership
• Graduate of an accredited nursing program or hold a BSN
• Knowledge of State and Federal regulations, hospital information systems, and staff development techniques
• Strong communication, critical thinking, and judgment skills
• Certification in Case Management is preferred
