About the role
Oversee and lead the daily operations of the Care Coordination department, ensuring high-quality, patient-centered care transitions. Manage and mentor Care Coordination RNs, monitor performance, ensure regulatory compliance, and collaborate with interdisciplinary teams to optimize patient flow and effectiveness.

Requirements
- Bachelor of Nursing upon hire or 1-3 years equivalent experience
- Minimum of three (3) years experience in Care Coordination or Case Management
- Registered Nurse: CA (RN:CA) required
- Certified Case Manager (CCM) preferred

$54.76 - $81.45 per hour
