RN Care Manager - Care Coordination

About the Role

This position involves facilitating payor authorization, managing patient care across the continuum, collaborating with multidisciplinary teams, ensuring documentation accuracy, resolving system and discharge barriers, and monitoring patient progress to deliver high-quality, efficient, and cost-effective care.

Responsibilities

• Manage authorization processes for patient care
• Coordinate patient care across the continuum
• Ensure accuracy of clinical documentation

• Monitor patient progress and optimize length of stay

Requirements

• Current Illinois nursing license
• Bachelor’s degree in Nursing (preferably from an NLN-accredited school)
• Minimum of 2 years of clinical nursing experience

Qualifications

• Experience in Utilization Management, Managed Care, or chronic disease management (preferred)
• Familiarity with Electronic Medical Records (EMR)
• Knowledge of InterQual or MCG criteria (preferred)
• Clinical certification (beneficial)
