RN Care Manager - Care Coordination

We are seeking a qualified RN Care Manager to facilitate patient care coordination, payor authorization, and documentation accuracy. Responsibilities Include Managing patient care across the continuum and resolving system barriers to ensure timely treatment and reimbursement.Collaborating with physicians, nursing staff, and multidisciplinary teams to promote patient progression.Monitoring patient progress, intervening when necessary, and addressing delays to optimize length of stay.Working with the CDI team to ensure documentation reflects illness severity and service intensity.Facilitating discharge planning and addressing systemic issues hindering diagnostics or treatment. Qualifications And Skills Current professional Nursing Licensure in Illinois.Bachelor’s degree in Nursing (preferred from an NLN-accredited school).Minimum of 2 years clinical nursing experience; previous utilization management, managed care, or chronic disease management experience preferred.Knowledge of EMR systems and criteria like InterQual or MCG preferred.Certifications such as case management or BLS/CPR are beneficial.
