RN Care Manager - Care Coordination

About the role

We are seeking a qualified RN Care Manager to facilitate patient care coordination, payor authorization, and documentation accuracy.

Responsibilities

• Managing patient care across the continuum and resolving system barriers to ensure timely treatment and reimbursement.
• Collaborating with physicians, nursing staff, and multidisciplinary teams to promote patient progression.
• Monitoring patient progress, intervening when necessary, and addressing delays to optimize length of stay.
• Working with the CDI team to ensure documentation reflects illness severity and service intensity.
• Facilitating discharge planning and addressing systemic issues hindering diagnostics or treatment.

Qualifications and skills

• Current professional Nursing Licensure in Illinois.
• Bachelor's degree in Nursing (preferred from an NLN-accredited school).
• Minimum of 2 years clinical nursing experience; previous utilization management, managed care, or chronic disease management experience preferred.
• Knowledge of EMR systems and criteria like InterQual or MCG preferred.
• Certifications such as case management or BLS/CPR are beneficial.

Schedule
