Position Details
Location: Norfolk, VA
Schedule: Monday‑Friday, 8:00 AM – 5:00 PM
Contract Length: 13 weeks
Patient Load: Approximately 5‑8 home visits per day
Requirements
- Minimum 1 year of recent Home Health (or Hospice) experience
- Active VA LPN/LVN License
- BLS/CPR Certification
- Recent HomeCare HomeBase (HCHB) experience
- OASIS experience preferred
- Reliable transportation and willingness to travel within the assigned territory
