About the Role

The LPN/LVN - Home Health provides quality patient care in both nursing centers and patients' homes, under the supervision of a Nurse Case Manager or Clinical Supervisor. The role involves performing nursing duties in compliance with state and federal regulations, with 25% to 35% of time dedicated to travel in the designated areas.

Responsibilities

• Deliver patient care following established nursing processes and procedures.
• Travel to patients' homes and nursing centers to provide care.
• Ensure compliance with regulatory standards and documentation requirements.
• Collaborate with healthcare team members to coordinate care.

Requirements

• Valid Oregon LPN license.
• BLS certification within 30 days of hire.
• At least one year of experience in acute care, home health, or related settings.
• Proof of driver’s license and auto insurance upon request.

Qualifications & Skills

• Licensed Practical Nurse certification in Oregon.
• Experience in patient care settings such as acute care or home health.
• Strong communication and organizational skills.
• Ability to travel as required (25% to 35% of time).
• Adherence to safety and regulatory standards.
