About the role

Provides quality nursing care to patients in their homes and nursing centers under the supervision of a Nurse Case Manager or Clinical Supervisor. Performs duties aligned with the nursing process, ensuring compliance with all state and federal regulations, including the Nurse Practice Act. The role involves 25% to 35% travel to patient locations within designated areas, delivering patient-centered, whole-person care that emphasizes understanding, commitment, and mutual respect.

Responsibilities

• Provide direct patient care in home and facility settings.
• Follow established nursing protocols and procedures.
• Ensure compliance with regulatory standards.
• Travel to patient homes within coverage areas.

Requirements

• Oregon Licensed Practical Nurse certification/license.
• BLS certification from American Heart Association (within 30 days of hire).
• Minimum of 1 year experience in acute care, home health, public health nursing, or a skilled nursing facility.
• Valid driver’s license and auto insurance, with the ability to travel as required.
