LVN - Full Time/Part Time (Day Shifts)

About the role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings, under the supervision of a Registered Nurse. The role involves delivering, documenting, and coordinating patient care according to physician orders and care plans.

Responsibilities

• Assessing patient conditions and recognizing changes requiring intervention
• Educating patients and families
• Ensuring compliance with safety and infection control standards
• Delivering, documenting, and coordinating patient care per physician orders and care plans
• Maintaining professional accountability and adhering to regulatory guidelines
• Participating in ongoing education

Requirements

• Active, unencumbered LPN/LVN license in the practice state
• Current CPR certification
• TB test within the last 12 months
• Reliable transportation
• Preferred: six months of clinical experience; home health experience is a plus

Skills

• Attention to detail and strong organizational skills
• Effective communication and problem-solving abilities
• Ability to lift up to 50 pounds and respond to emergencies
• Capable of working in diverse environments with exposure to infectious materials
• Proficiency in English, ability to travel, and physical capability for prolonged walking, standing, and repositioning patients
• Ability to work in varied settings, including exposure to blood and bodily fluids
