LVN - Full Time/Part Time (Day Shifts)

About the role

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings, under the supervision of a Registered Nurse, ensuring quality patient outcomes.

Responsibilities

• Delivering and documenting nursing care according to physician orders and care plans.
• Monitoring and assessing patient conditions, reporting changes promptly.
• Updating and implementing nursing care plans, taking appropriate actions to meet patient needs.
• Educating patients and families on disease management, self-care, and prevention.
• Maintaining compliance with policies, regulations, and infection control standards.

Qualifications

• Active, unrestricted LPN/LVN license in the practice state.
• Current CPR certification and TB test within the last 12 months.
• Proficiency in clinical assessments, documentation, and compliance.
• Strong attention to detail, time management, problem-solving, and communication skills.
• Must have reliable transportation and physical ability to lift 50 pounds, reposition patients, and perform mobility tasks.

Additional requirements

• Ability to travel and operate effectively in varied environments.
• Compliance with health and safety standards, including exposure to potential infectious materials.
• Ongoing education, adherence to evolving regulations, and may include additional duties as needed to ensure exceptional patient care.

Preferred

• Six months of recent clinical experience, home health experience a plus.
