LVN - Full Time/Part Time (Day Shifts)

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings. Responsibilities Include Delivering and documenting patient care according to care plans and physician ordersMonitoring and assessing patient conditions, reporting changes promptlyUpdating and implementing nursing care plansAdministering appropriate nursing interventions to achieve desired outcomesEnsuring infection control standards and patient safety measuresEducating patients and families on disease management and self-care techniquesMaintaining compliance with policies, regulations, and professional standards Qualifications And Skills Active, unrestricted LPN/LVN license in the practice stateCurrent CPR certificationTB test within the last 12 monthsPreferred: six months of recent clinical experience; home health experience is a plusReliable transportation and physical ability to lift 50 pounds and reposition patientsStrong attention to detail, organization, communication, and problem-solving skills Additional Requirements Ability to work in diverse environments with exposure to bodily fluids and infectious diseasesProficiency in assessments, documentation, and compliance with nursing policiesAbility to travel as needed and respond effectively to emergencies This role emphasizes compassionate, patient-centered care, with opportunities for skill development and flexible scheduling.
