About the role

Delta Health Center, Inc. (DHC) is the first community health center of its kind and the first Federally Qualified Community Health Center (FQHC) in the United States. DHC is a non-profit organization located in historic Bolivar County, Mississippi, where it opened its doors in 1965. We are committed to caring for vulnerable populations from newborns to the elderly, helping generations of families to live and stay healthy.

Responsibilities

• Triage Patients
• Expedite patient referrals to specialty offices
• Answer patient and specialty office calls
• Complete referral requests
• Maintain ongoing tracking and documentation on referrals
• Work independently and unsupervised
• Ensure that referrals are addressed timely
• Remind patients of their appointments

Requirements

• Licensed Practical Nurse Degree: Certificate of Completion
• Must be IV certified
• General Computer Skills Required: Microsoft Outlook and Excel

Qualifications

• Completed License Needed: Upon starting work
• Experience: Healthcare: 1 year (Preferred)
• Nursing: 1 year (Preferred)
• Licensed Practical Nurse (Required)
