LPN - Physician Practice (Full & Part-Time, Sign-On Bonus Available)

About the Role

As a Licensed Practical Nurse, you will be an integral part of the patient care team, providing outpatient nursing services. You will utilize the nursing process to deliver care to both healthy and health-challenged patients, collaborating with community resources to promote, restore, and maintain health.

Responsibilities

• Document all patient care activities accurately
• Collect and share patient data, including vital signs, history, and chief complaints
• Assist in preparing patients for procedures, tests, and treatments
• Support healthcare providers in assessing patient needs and problems
• Identify risk factors related to age, medications, and physical functions
• Evaluate supplies, equipment, and facility readiness

Requirements

• Graduation from an accredited LPN program and current NY State license
• Infection control and BLS certifications
• Strong communication skills and ability to perform basic math
• Ability to provide care under supervision
• Ability to recognize and respond to patient care emergencies
• Commitment to ongoing professional development and staying current with nursing practices
