Responsibilities
- Schedules home visits for field staff and coordinates patient care.
- Schedules and communicates with field staff, patients, and Clinical Manager regarding scheduling of patient services.
- Ensures patient assessment visits including all Oasis visits are scheduled and performed timely.
- Prepares weekly clinician schedules and reviews them daily or more frequently as needed.
- Schedules clinicians in the most efficient geography, maximizing clinician efficiency and utilizing the lowest possible discipline.
- Schedules missed visits by the end of the current week or refers unresolved issues to the Clinical Manager.
- Collaborates with field clinicians to ensure all patient visits are scheduled and completed as ordered.
- Manages calls from patients and field staff related to scheduling issues and refers clinical and performance related issues to the Clinical Manager.
- Maintains contact with hospitalized patients, ensuring the team follows up as necessary.
- Completes requested schedules ensuring all orders and services are included as required.
- Supports Clinical Manager and collaborates to ensure timely communication with patients, clinicians, referral sources, BD team, and other office personnel.
- Runs applicable reports and takes appropriate actions based on workflow.
- Assists with internal or external transfers of patients between care centers and/or Hospice services.
- If a licensed clinician, may provide direct patient care in emergencies.
- If a licensed clinician, may maintain supply closet with routine supplies, ensure supplies are within expiration dates, and serve as liaison with the field for patient supply needs.
- If a licensed clinician, may obtain verbal orders for patients and document as applicable.
- Performs other duties as assigned.
Qualifications
- Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse specific to the state of employment.
- A high school diploma or GED equivalent.
- One (1+) year of administrative experience in a healthcare environment.
- One (1+) year of scheduling experience in a healthcare environment (preferred).
