About the Role

The Licensed Practical Nurse (LPN) provides general nursing care to patients, families, and caregivers in the Medical Group practices. The LPN assists and advocates for patient and family health in support of a patient-centered approach and delivery model of care, acting as an active care team member. Responsibilities include collaborating with physicians, providers, registered professional nurses, and multidisciplinary care team members.

A Licensed Practical Nurse performs tasks within the framework of case finding, health coaching, and supportive care under the direction of a registered professional nurse or licensed physician or healthcare provider. Nursing duties align with standards outlined in the NYS Education Law, NYS Office of the Professions, and the State Nurse Practice Act. The LPN adheres to the mission, vision, I-CARE values, policies, and procedures of Mohawk Valley Health System.

Responsibilities

  • Gather patient measurements and collect patient data.
  • Record and report patient findings.
  • Prepare patients for examination and treatment.
  • Assist with examinations and treatments.
  • Perform tasks within the framework of supportive care under the clinical direction of a licensed healthcare provider or registered professional nurse.
  • Support self-care, self-management, and the established plan of care.
  • Utilize standard precautions and safe practices at all times.
  • Maintain equipment, supplies, inventory, and a safe, clean work environment.
  • Maintain technical knowledge by attending educational workshops, mandatory in-services, reviewing publications, and participating in department committee meetings or workgroups.
  • Engage in quality improvement initiatives, including continuing education and participation in system-wide and department-specific teams.
  • Maintain cooperative relationships among the healthcare team through effective communication.
  • Respond to patient requests and communicate effectively to the licensed healthcare provider.

Requirements

  • Graduate of an accredited school of licensed practical nursing program (required).
  • Bachelor’s or Master’s of Science in Nursing from an accredited nursing program (preferred).
  • Current New York State licensure as a Licensed Practical Nurse (required).
  • Basic Life Support (BLS) Certification – must receive within 6 weeks of hire date (required).

Pay

$22.00 - $32.00 per hour.

  • FTE: 1 (Full-Time)
  • Work Schedule: HRLY NON-UNION-8 HR
