About the Role

Ensure safe, accurate medication delivery, monitor resident health, and uphold regulatory standards to protect resident safety. The role involves clinical precision, timely documentation, and vigilant observation of residents' health changes.

Responsibilities

• Administer medications and treatments following physician orders
• Follow the Six Rights of Medication Administration
• Accurately document in MAR/TAR in real-time
• Monitor symptoms, report adverse reactions, and conduct health updates
• Transcribe physician orders and support care plan updates
• Observe and report physical, emotional, or cognitive changes
• Maintain medication carts, perform narcotic counts, and ensure compliance

Requirements

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills
• Experience in senior care or medication administration preferred
