LPN Medical Care Manager

About the role

We are seeking an LPN Medication Nurse responsible for providing safe and accurate medication administration, health monitoring, and precise clinical documentation. This role emphasizes resident safety, regulatory adherence, and clinical precision to prevent medication errors and identify health changes early.

Responsibilities

• Administering medications and treatments according to physician orders
• Following the Six Rights of Medication Administration
• Documenting in MAR/TAR accurately and in real time
• Monitoring symptoms and administering medications for relief
• Reporting any refusals or adverse reactions promptly
• Conducting Service & Health Updates (SHUs)
• Transcribing physician orders and supporting care plan updates
• Observing and reporting physical, emotional, or cognitive changes
• Maintaining medication carts, completing narcotic counts, and ensuring compliance standards

Requirements

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills

Qualifications

• Experience in senior care or medication administration preferred
