LPN Medical Care Manager

About the Role

The LPN Medication Nurse provides licensed nursing support in a senior living setting, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

Responsibilities

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Document accurately in MAR/TAR in real time
• Monitor symptoms, report adverse reactions, and support care plan updates
• Observe and report physical, emotional, or cognitive changes in residents
• Maintain medication carts, conduct narcotic counts, and ensure compliance with regulatory standards
• Transcribe physician orders and conduct Service & Health Updates (SHUs)
• Ensure medication safety, early detection of health changes, and resident safety through clinical precision and regulatory adherence

Requirements

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills
• Experience in senior care or medication administration preferred
