About the role

As an LPN Long Term Care Facility Nurse, you will play a vital role in delivering exceptional patient care and promoting independence, health, and well-being.

Responsibilities

• Provide direct patient care to residents, including administering medications, performing vital signs, and conducting routine assessments
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Conduct routine rounds to monitor patient progress and identify areas for improvement
• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
• Participate in quality improvement initiatives to enhance patient outcomes and facility operations

Requirements

• Absolutely must be a Licensed Practical/Vocational Nurse (LPN/LVN) with a current license to practice in the state of Ohio
• Strong communication, interpersonal, and problem-solving skills
• Ability to work effectively in a fast-paced, dynamic environment
• Strong attention to detail and ability to maintain accurate records
• Compassionate and empathetic approach to patient care
• A passion for delivering exceptional patient outcomes and making a positive impact in the lives of others

Qualifications

• Must have a valid driver's license and reliable transportation
• Must be able to lift up to 50 pounds
• Must have a valid Social Security number
