LPN for Long Term Care Facility

About the role

Responsible for providing direct patient care in a long-term care facility setting. Duties include administering medications, assisting with daily living activities, and monitoring patient health.

Responsibilities

• Administer medications as prescribed
• Assist patients with bathing, dressing, eating, and other daily activities
• Maintain accurate medical records and documentation
• Monitor and report changes in patient condition
• Participate in team meetings and training sessions

Requirements

• High school diploma or equivalent
• Current certification as a Licensed Practical Nurse (LPN)
• Valid driver’s license and access to reliable transportation
• Experience in long-term care or related field preferred

Qualifications

• Strong communication skills
• Ability to work independently and as part of a team
• Physical stamina for lifting and moving patients

Skills

• Proficient in computer systems
• Knowledge of long-term care protocols and procedures

Schedule
