About the role
As an LPN at Luna Rehabilitation & Care Center, you will provide direct nursing care, administer medications and treatments, monitor resident conditions, supervise nursing assistants, and collaborate with the interdisciplinary team to ensure safe, timely, and individualized care. This role is ideal for both experienced nurses and new graduates looking to grow their clinical skills in a skilled nursing environment.
Responsibilities
- Provide direct nursing care in accordance with individualized care plans, physician orders, facility policies, and established nursing standards.
- Administer prescribed medications, treatments, and therapeutic interventions within the LPN scope of practice.
- Monitor residents for changes in physical, cognitive, emotional, and behavioral condition and promptly report significant findings.
- Obtain and monitor vital signs and other clinical information as required.
- Notify the appropriate RN, physician, provider, or nursing leadership of significant changes in resident condition.
- Monitor resident responses to medications, treatments, and nursing interventions.
- Assist with admissions, transfers, discharges, and changes in resident condition as assigned.
- Collaborate with RNs, physicians, CNAs, therapy, dietary, social services, and other interdisciplinary team members to coordinate resident care.
- Supervise, delegate, and provide direction to CNAs and other nursing support personnel within applicable scope and facility policy.
- Ensure nursing assignments and resident-care responsibilities are completed appropriately and timely.
- Provide residents and families with education regarding medications, treatments, and care needs.
- Maintain accurate, complete, and timely documentation within the electronic medical record (EMR).
- Respond promptly and appropriately to urgent resident needs and emergency situations.
- Follow infection prevention and control practices, including hand hygiene, PPE requirements, and applicable isolation precautions.
- Protect resident rights, privacy, dignity, confidentiality, and personal preferences.
- Maintain compliance with facility policies and applicable federal and New Mexico requirements governing skilled nursing and resident care.
Requirements
- Graduate of an approved practical nursing program.
- Current, active Licensed Practical Nurse (LPN) license in good standing in the State of New Mexico.
- Current CPR or Basic Life Support (BLS) certification preferred or required according to facility policy.
- New graduate LPNs are welcome and encouraged to apply.
- Previous skilled nursing, long-term care, rehabilitation, or healthcare experience preferred but not required.
- Strong clinical observation and resident assessment skills within the LPN scope of practice.
- Ability to recognize and appropriately communicate changes in resident condition.
- Strong organizational and time-management abilities.
- Excellent communication and interpersonal skills.
- Ability to provide direction and support to nursing assistants and work collaboratively with licensed nursing personnel.
- Ability to manage multiple responsibilities in a fast-paced clinical environment.
- Compassionate and professional approach to caring for older adults and individuals with complex healthcare needs.

Pay
$34.00–$38.50 per hour, with a $2,500 Sign-On Bonus for eligible Full-Time Nurse Positions.
