About the role

Cedar Ridge Inn is seeking a compassionate, dependable, and clinically skilled Licensed Practical Nurse (LPN) to join our nursing team. As an LPN, you will provide direct nursing care, administer medications and treatments, monitor resident conditions, and support the delivery of safe, high-quality care within our skilled nursing environment. You will work closely with RNs, CNAs, physicians, and other members of the interdisciplinary team to ensure each resident receives individualized and person-centered care.

Responsibilities

  • Provide direct nursing care in accordance with individualized care plans, physician orders, professional nursing standards, and facility policies
  • Monitor and assess residents within the LPN scope of practice and promptly identify changes in physical, cognitive, behavioral, or psychosocial condition
  • Communicate significant changes in resident condition to the appropriate RN, physician, nursing leadership, and responsible party
  • Safely prepare and administer prescribed medications, treatments, and therapeutic interventions within scope of practice
  • Monitor resident responses to medications and treatments and report concerns or adverse reactions appropriately
  • Coordinate with physicians, RNs, CNAs, pharmacy, therapy, dietary, social services, and other interdisciplinary team members to promote continuity of care
  • Assist with admissions, readmissions, transfers, and discharges as assigned
  • Provide residents and families with appropriate education regarding medications, treatments, care plans, and health management
  • Supervise, delegate, and provide direction to CNAs and other nursing support personnel within applicable scope-of-practice requirements
  • Ensure assigned resident care is completed safely, timely, and according to individualized care plans
  • Maintain accurate, complete, and timely clinical documentation within the electronic medical record (EMR)
  • Follow up on physician orders, laboratory results, treatment recommendations, and other clinical information as assigned
  • Participate in infection prevention, resident safety, quality assurance, and performance improvement initiatives
  • Respond promptly and appropriately to urgent resident needs and emergency situations
  • Protect resident rights, dignity, privacy, confidentiality, and individual preferences
  • Maintain compliance with facility policies and applicable federal and New Mexico requirements governing skilled nursing care

Requirements

  • Graduate of an accredited practical nursing program
  • Current, active Licensed Practical Nurse (LPN) license in good standing in the State of New Mexico
  • Current CPR or Basic Life Support (BLS) certification
  • Skilled nursing, long-term care, rehabilitation, or post-acute nursing experience preferred
  • Strong clinical observation, critical-thinking, and decision-making abilities within the LPN scope of practice
  • Ability to recognize changes in resident condition and appropriately escalate clinical concerns
  • Ability to effectively supervise, delegate, and support nursing assistants
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management abilities
  • Ability to prioritize multiple resident needs in a fast-paced clinical environment
  • Ability to collaborate effectively with residents, families, physicians, and interdisciplinary team members

Physical Requirements

  • Ability to assist with lifting, transferring, repositioning, and mobility needs of residents using appropriate assistance and equipment
  • Ability to stand, walk, bend, reach, and perform nursing duties for extended periods
  • Ability to conduct resident rounds and provide direct nursing care throughout the facility
  • Ability to safely perform nursing procedures and operate applicable clinical equipment
  • Ability to respond promptly and effectively to changes in resident condition and emergency situations
  • Ability to perform the essential functions of the position with or without reasonable accommodation
