About the role

Ennoble Care is looking for a full-time Hospice Licensed Practical Nurse to work within and around the West Philadelphia, PA area. The role involves providing care and support to terminally ill patients, assisting the Registered Nurse in carrying out the plan of care, performing specialized procedures, preparing and implementing the patient plan of care, and collaborating with the interdisciplinary group to address patient needs.

Responsibilities

  • Assist the Registered Nurse in carrying out the plan of care.
  • Auxiliary in performing specialized procedures (as defined in the State Nurse Practice Act) as directed by the Registered Nurse under physician's orders.
  • Implement and evaluate the patient plan of care actively.
  • Collaborate with the IDG to keep the patient’s plan of care current and address any identified issues.
  • Reinforce teaching to the patient/family members regarding end of life care.
  • Foster cooperative efforts among personnel by understanding and participating in team and staff conferences.

Requirements

  • Graduated from an accredited School of Practical Nursing.
  • Licensed in VA as a Licensed Practical/Vocational Nurse by Virginia State Board of Nursing.
  • At least one year of nursing experience, with hospice experience preferred.
  • Knowledgeable in caring for the terminally ill, including administering medication and providing emotional support.
  • Driver with an insured automobile in good working order.
  • The ability to communicate well, both verbally and in writing.
  • Proven experience as a hospice LPN.
  • Committed to safety precautions and high ethical standards.
  • Sound judgment and attention to detail.
  • Excellent communication skills, bilingual a plus.
  • Compassionate and patient with an emotional stability.

Qualifications
