Licensed Practical Nurse / LPN

About the role

Easterseals PORT Health (ESPH) is seeking compassionate and motivated full-time Licensed Practical Nurses to join our Virginia REACH team. The role supports individuals with Intellectual/Developmental Disabilities (I/DD) and co-occurring behavioral health needs at our Crisis Therapeutic Home (CTH) in Staunton, VA.

Responsibilities

• Provide compassionate medical and nursing support to individuals with I/DD and co-occurring behavioral health needs
• Administer medications safely and accurately in accordance with clinical protocols
• Conduct nursing assessments
• Educate and empower individuals regarding their mental and physical health
• Collaborate closely with interdisciplinary team members to ensure coordinated, person-centered care
• Maintain thorough and timely documentation of all nursing services and observations
• Uphold compliance with all regulatory and clinical care standards to ensure the highest quality of service

Requirements

• Current valid, unrestricted licensed as a Practical Nurse in state of Virginia
• Knowledge of MH and I/DD systems of care preferred
• Strong communication and documentation skills
• Experience in crisis prevention and intervention services preferred

$26.00 - $31.00 per hour for this full time, non-exempt position

• Monday - Friday, 3pm-11pm
• Monday - Friday, 11pm-7am
