About the role

The Licensed Practical Nurse (LPN) Navigator guides patients through complex care pathways, focusing on access, referral accuracy, sequencing, and continuity across services and settings.

Responsibilities

• Review and triage referrals for appropriateness and urgency, ensuring necessary records are obtained.
• Collaborate with healthcare providers to coordinate and implement individualized care plans.
• Communicate effectively with patients via mail, phone, and in person, providing education as needed.
• Serve as a primary contact for providers and coordinate patient activities like support groups and educational series.
• Support internal referral retention, monitor patient follow-up, and maintain systems for timely surveillance.
• Investigate patient concerns related to access, scheduling, and billing, and review medical records for physician reporting.
• Track patient progress from referral to care completion, helping navigate system barriers.

Qualifications

• Graduate from a Specialty Training Program in Nursing.
• At least 2 years of LPN experience.
• Strong clinical knowledge, communication skills, and care coordination abilities.
• Adherence to policies and a focus on quality improvement.
