About the role
This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.
Responsibilities
- Provide care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and Assisted Living Facilities.
- Collaborate daily with a team of clinicians.
- Work closely with patients on a 1-1 level but get strong support from a patient care team.
- Work independently and on your own from a day-to-day basis with activities which include phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of state and federal rules and regulations.
- Manage visits, documentation, meetings (i.e. to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns).
- Learn an electronic medical records system.
- Be willing to drive and reliable transportation, valid driver’s license, and auto insurance.
Requirements
- Current Licensed Practical Nurse (LPN) licensure (in good standing) in the state(s) of practice with one year of experience under license.
- Ability to work well within a team environment to collaborate daily with a team of clinicians.
