Licensed Practical Nurse - (LPN )

Responsibilities

• Provide care and services to a group of patients, visiting them at their residences including private homes, Skilled Nursing Facilities, and Assisted Living Facilities.
• Contribute to the interdisciplinary team, offering expertise in psychosocial assessments, interventions, and knowledge of community resources.

Requirements

• Current Licensed Practical Nurse (LPN) licensure (in good standing) in the state(s) of practice with one year of experience under license.
• Ability to work well within a team environment, collaborating daily with a team of clinicians.
• Work closely with patients on a 1-1 level, receiving strong support from a patient care team.
• Work independently and on your own, managing activities such as phone, email, and written communication, attending meetings with various healthcare professionals, and understanding state and federal rules and regulations.
• Manage visits, documentation, and meetings effectively, prioritizing and triaging multiple concerns.
• Be willing to drive and maintain reliable transportation, possess a valid driver’s license, and have auto insurance.

Qualifications

Must be able to learn an electronic medical records system.
