Licensed Practical Nurse - (LPN )

About the role

The Licensed Practical Nurse (LPN) at Brighton/Suncrest Hospice will provide care and services to a group of patients, visiting them in various settings such as private homes, Skilled Nursing Facilities, and Assisted Living Facilities. This role is a contributing member of the interdisciplinary team, providing expertise in psychosocial assessments, interventions, and knowledge of community resources.

Responsibilities

• Provide care and services to patients in various settings
• Collaborate with the interdisciplinary team to offer expertise in psychosocial assessments and interventions
• Work independently and on a 1-1 level with patients, supported by a patient care team
• Manage visits, documentation, and meetings efficiently
• Manage time effectively to prioritize and triage multiple concerns
• Learn and utilize an electronic medical records system
• Drive and maintain reliable transportation with a valid driver's license and auto insurance

Requirements

• Current Licensed Practical Nurse (LPN) licensure in the state(s) of practice with one year of experience under license
• Ability to work well within a team environment and collaborate daily with a team of clinicians
• Strong communication skills, including phone, email, and written communication
• Understanding of state and federal rules and regulations related to healthcare
