Role Overview

The Licensed Practical Nurse (LPN) provides professional nursing care to residents in a long-term care setting, under supervision.

Key Responsibilities

  • Assess residents' health status, administer medications and treatments as ordered, and evaluate data to monitor resident well-being.
  • Contribute to care planning, recognize issues, recommend solutions, and respond effectively in emergent situations.
  • Ensure residents' safety, comfort, and overall health; document interactions, outcomes, and observations related to care, medication, behavior, activity, and diet.
  • Provide technical support to healthcare professionals and communicate clearly with residents, families, and team members.
  • Assist in resident education, developing self-care programs, and counseling on prevention and treatment plans.
  • Collaborate as part of an interdisciplinary team, maintaining confidentiality and establishing effective working relationships.
  • Perform additional duties as assigned, adhering to clinical standards and department policies.

Qualifications

  • Graduate from an accredited practical nursing program; associate degree preferred.
  • Hold an unencumbered LPN license with multi-state privileges.
  • Possess IV therapy skills if required, with relevant coursework completed.
  • Complete supervised clinical experience or minimum of 400 hours as an LPN.
  • Maintain required certifications, competencies, and continuing education units annually.
