Key Responsibilities
- Provide direct nursing care to residents in accordance with care plans
- Administer medications and treatments as prescribed
- Monitor and document residents’ health and report changes to the care team
- Collaborate with nurses, CNAs, and interdisciplinary team members to ensure quality resident care
Qualifications
- Current LPN license in Pennsylvania
- Compassionate, reliable, and team-oriented
- Strong communication and clinical skills
- Previous long-term care experience preferred but not required
