(LPN) Licensed Practical Nurse

About the Role

Located in Phillipsburg, New Jersey, Complete Care at Brakeley Park is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team of healthcare professionals. As an Evening Shift LPN, you will provide high-quality, individualized care to our residents while fostering a warm and supportive environment.

Responsibilities

• Provide direct patient care to a diverse population of seniors, utilizing your knowledge and skills to assess, plan, and implement individualized care plans
• Administer medications, treatments, and therapies as prescribed by physicians and other healthcare professionals
• Collaborate with interdisciplinary teams to ensure comprehensive and coordinated care
• Monitor and report patient status to nursing staff, physicians, and other healthcare professionals
• Participate in quality improvement activities to enhance patient outcomes and satisfaction
• Develop and maintain positive relationships with residents, families, and coworkers

Qualifications

• Current LPN licensure in the state of New Jersey
• Strong clinical skills and ability to work effectively in a fast-paced environment
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work varying shifts, including evenings and weekends
• Commitment to providing exceptional patient care and promoting a positive work environment
