Complete Care at Barn Hill is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team! As an LPN, you will work closely with our interdisciplinary team to develop and implement individualized care plans, monitor patients' conditions, and update medical records. Responsibilities

• Provide direct patient care, including administering medications, monitoring vital signs, and performing routine tasks
• Assess patients' conditions, identifying areas of concern and reporting to the healthcare team
• Develop and implement individualized care plans, in collaboration with the interdisciplinary team
• Monitor and maintain accurate medical records, ensuring timeliness and accuracy
• Maintain a clean and organized environment, adhering to infection control protocols
• Collaborate with other healthcare professionals to ensure seamless care transitions
• Participate in quality improvement initiatives, contributing to the growth and development of our healthcare services

Requirements

• Licensed Practical Nurse (LPN) license in the state of New Jersey
• Strong communication, interpersonal, and teamwork skills
• Ability to work effectively in a fast-paced, dynamic environment
• Compassionate and empathetic approach to patient care
• Basic computer skills, including electronic medical records

Pay

Competitive hourly rate of $34.00 – $41.75, plus shift differentials.

Schedule

• Evenings - Full Time with Paid Time Off (PTO)
• Overnight - Part Time
