LPN - Intermittent Visits - Evening

Role: Licensed Practical Nurse (LPN) providing one-on-one homecare visits to patients requiring intermittent care. Responsibilities Utilize the nursing process to plan, implement, and evaluate patient care based on diagnosis and goals.Initiate and revise care plans as patient needs change, in coordination with prescribers.Administer medications and treatments, monitor effectiveness, and document outcomes.Respond to changes in patient condition, report to physicians or supervisors, and support discharge planning.Follow professional standards, legal requirements, and organizational policies.Participate in team meetings, case conferences, and ongoing education. Qualifications & Skills Graduate of an accredited nursing school.Current LPN license for the relevant state.Current BLS certification, PPD or Chest X-Ray.Preferably one year of LPN experience.Adherence to professional standards, laws, and patient rights. Key Duties Perform catheter, wound, diabetic, and medication management during visits typically lasting 30 minutes to 1 hour.Support patient education, care coordination, and adherence to healthcare protocols.Maintain compliance with all legal, organizational, and professional guidelines.
