Job Summary
Kadima Rehabilitation & Nursing at North Strabane is seeking a dedicated and compassionate Evening LPN to join their team at North Strabane Rehabilitation and Wellness Center in Canonsburg, PA.
Responsibilities
- Provide direct patient care, including vital signs, wound care, and administration of medications as prescribed
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Conduct routine rounds to monitor resident condition and report any changes or concerns to the nurse manager
- Maintain accurate and detailed records of patient care and treatment
- Participate in quality improvement initiatives and staff development programs
- Demonstrate professionalism, compassion, and respect for residents, families, and colleagues
