Role: Registered Nurse (RN) providing one-on-one intermittent homecare visits to patients. Responsibilities Utilize the nursing process to plan, implement, and evaluate patient care based on individual needs.Develop and revise care plans in collaboration with prescribers as patient conditions change.Administer medications and treatments, monitor their effectiveness, and document outcomes.Respond to changes in patient condition by intervening appropriately and reporting to physicians or supervisors.Educate patients and support care coordination, discharge planning, and goal setting.Ensure compliance with professional standards, legal requirements, and organizational policies.Participate in team meetings, case conferences, and ongoing education activities.Perform additional duties as assigned. Qualifications & Skills Graduate of an accredited nursing program.Current RN license for the relevant state.Valid BLS certification and current PPD or Chest X-Ray.Preferred but not mandatory: one year of RN experience.Adherence to professional standards, practice, and legal regulations.
