About the role
Licensed Practical Nurse (LPN) providing skilled nursing care in patients' homes.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services according to prescribed orders.
- Monitor signs and symptoms to identify physiologic and psychosocial changes.
- Collect, analyze, and interpret health data; document nursing diagnoses and patient information.
Qualifications
- Current LPN/LVN license in the practicing state.
- Valid TB or Chest X-Ray and BLS certification.
- Preferred: At least one year of professional nursing experience.
Key Duties
- Conduct hourly skilled nursing visits such as diabetic management, wound care, medication administration, and catheter changes in patients' homes.
