LPN Homecare - Skilled Nursing Visits

Role: Licensed Practical Nurse (LPN) providing skilled nursing care in patients' homes. Responsibilities Assess, plan, implement, and evaluate patient care using the nursing process.Deliver skilled nursing services according to prescribed orders.Monitor signs and symptoms to identify physiologic and psychosocial changes.Collect, analyze, and interpret health data; document nursing diagnoses and patient information. Qualifications Current LPN/LVN license in the practicing state.Valid TB or Chest X-Ray and BLS certification.Preferred: At least one year of professional nursing experience. Key Duties: Conduct hourly skilled nursing visits such as diabetic management, wound care, medication administration, and catheter changes in patients' homes. Skills: Strong assessment, clinical, and documentation skills; compassionate patient care; ability to work independently.
