Home Care LPN Visits
About the role
To enable patients to stay independent in their homes, Licensed Practical Nurses (LPNs) perform routine supervisory visits, evaluate the plan of care, and supervise excellent client service.
Responsibilities
- Evaluate the plan of care and supervise delivery of excellent client service
- Perform routine supervisory visits
- Communicate with Case Managers and other members of the care team
- Present educational in-service trainings
Requirements
- Graduation from an accredited school of nursing
- Certified LPN licensure
- One year of nursing experience
- Must be able to travel locally in your own vehicle, and possess a valid driver's license and appropriate automobile insurance coverage
