About the role
Licensed Practice Nurse (LPN) providing in-home patient care under supervision.
Responsibilities
- Assess, plan, and implement personalized care
- Educate patients and families on disease management, procedures, and medications
- Manage wound care, tracheostomy, G-tubes, catheters, ventilators, and medication administration
- Develop customized care plans to meet individual needs and preferences
Qualifications
- Valid LPN license
- Ability to perform clinical assessments
- Strong communication skills
- Willingness to work with diverse patient cases including pediatric and adult higher acuity cases
- New graduates are encouraged to apply with training support
